Please keep the following in mind before completing this form:

Pets are not available for off-site appearances. Event requests are for CHS staff and/or volunteers only.
Please submit your request at least 30 days in advance to allow enough time for planning and coordination.
Attendance is based on ambassador availability and cannot be guaranteed.
A member of the team will respond within 3–5 business days.
Event dates cannot be reserved or held until this request form has been submitted and reviewed.
